Feeling ill? Unsure about where to go to get the care that you need? Use the information below to help you find the information you need to get the right care, at the right time, and in the right setting.

PRIMARY CARE PROVIDER (PCP)

Call and make an appointment with your PCP for regular & preventive care, including:

  • – Checkups and shots
  • – Preventive care
  • – Fever, flu, sore throat, cough
  • – Infection
  • – Vomiting
  • – Injuries such as sprains or cuts
  • – Help with long-term illnesses such as asthma or diabetes

As a Community First Member, your PCP or a doctor on call is available to you at all times, including in the evenings and on weekends. You can find your PCP’s name and phone number listed on your Member ID card.

URGENT CARE

There are some injuries and illnesses that are probably not emergencies but can turn into emergencies if they are not treated within 24 hours.

When you have an urgent health care need, call your PCP first. They may tell you to go to an urgent care clinic. You do not need to call the clinic before going. Choose a network clinic that accepts Community First health insurance.

The Community First Nurse Advice Line is also available to you 24/7/365 at no cost. An experienced Registered Nurse can provide medical advice and information on what to do and where to go to care for a health care issue.

EMERGENCY ROOM

Dial 9-1-1 or go to the Emergency Room if you think your life is in danger.

The Emergency Room can provide fast, life-saving care for emergencies, including:

  • Bleeding that will not stop
  • Difficulty breathing
  • Seizures
  • Fainting
  • Chest pain
  • A serious accident

In the case of possible poisoning, call the Poison Control Center: 1-800-222-1222 for expert advice. They may direct you to the Emergency Room.
