Member Alert: Beware of Impersonation Scams

Community First Health Plans (Community First) has been made aware of reports that a company is contacting members, falsely claiming to be calling on behalf of Community First to provide service coordination and assessments. This company is not affiliated with Community First, nor has it been hired or authorized by us to contact members on our behalf.

Community First does not contract with external organizations to conduct or assist with member assessments or to provide service coordination. These services are provided exclusively by Community First employees, including Health Educators, Case Managers, and Service Coordinators.

While Community First occasionally partners with trusted health and wellness vendors, we always notify members in advance when an approved partner may contact them on our behalf. If you receive an unexpected call and are unsure of its legitimacy, please contact Community First directly at 1-800-434-2347 to verify before sharing any information.

How to Protect Yourself

Do not provide sensitive information such as:

  • Medical identification numbers or information
  • Social Security numbers
  • Online banking usernames, passwords, or PIN numbers
  • Debit or credit card numbers

Be cautious of pressure tactics. Scammers often create a sense of urgency and may encourage immediate action.

Verify before you trust. If you receive a suspicious call or message claiming to be from Community First, hang up and contact us directly using the phone number listed on our official website or on the back of your member id card.

Do not click suspicious links in text messages or emails and avoid downloading attachments from unknown senders.

Monitor your accounts regularly and report any unauthorized activity immediately.
