Thank you for your interest in becoming a partner with Community First Health Plans. CFHP’s ongoing assessment of its provider network against availability and access standards ensures that they have adequate services throughout the network. Please complete the following information below to begin the process of becoming a partner.
Our Programs
STAR / Medicaid
CHIP / CHIP Perinatal
Coverage for children under 19 years in families who earn too much to qualify for Medicaid but cannot afford private insurance. CHIP Perinatal provides prenatal care for expectant mothers and their unborn baby.
STAR Kids
A Texas Medicaid managed program that provides Medicaid benefits to children and adults under 20 who have special needs.
STAR+PLUS
The STAR+PLUS Program covers adults who have disabilities or are age 65 or older.
Aperture Credentialing, LLC is the Credentials Verification Organization (CVO) used by CFHP. Once verified and approved by CFHP, a provider must complete orientation where they will learn about important updates, information related to authorizations and claims, eligibility, and navigating the provider portal.
