Community First Provider Directories help you pick a provider for your health care needs.
It’s important that you choose a primary care provider (PCP). Your PCP will help you get your health care benefits. If you do not pick a PCP, one will be chosen for you.

 

You can call your PCP any time, day or night. Someone will always be available to take your call. If you are unable to reach your doctor and need help, call the Nurse Advice Line toll-free at 1-800-434-2347.

Community First does not require a referral to see a specialist. Some specialists may require a referral from your PCP before they will see you. Additionally, some services require preauthorization from Community First before you can get care. Your PCP will take care of this request for you.
You can also call Member Services to find out if a certain service requires preauthorization.

Some services that do not require a referral:

  • Behavioral health services
  • OB/GYN services
  • Vision exams from an optometrist

Community First Provider Directories, Member Handbooks, and Member Newsletters are available in print, free of charge. Call Member Services at 1-800-434-2347 or request a copy through the Member Portal, and we’ll mail you one within five business days.
