How to apply for no-cost and low-cost health coverage

At Community First Health Plans, our goal is to provide our community access to quality health care. It’s important to us that people understand that options ARE available. That’s why we have created an easy-to-follow Step-by-Step Guide to applying for health care benefits (available in English and Spanish).

This guide outlines 4 easy steps you can take to apply for coverage for your child, yourself, or other members of your family, if eligible.

Step 1

Learn about health care programs

Learn about different health plans offered through the Texas Health and Human Services Commission (HHSC), what health care services are included, and the cost of each plan.

Step 2

Determine who qualifies

Find out if you, your child, or other members of your family qualify for these health plans, and the income guidelines your family will need to meet in order to apply. (Guidelines are set by HHSC.)

Step 3

Apply for coverage

If you qualify, review the documents you will need to apply. Then, submit an online application to HHSC at YourTexasBenefits.com.

Pick the health plan that’s right for you

Once you are approved, you will receive a package from HHSC. You’ll then need to choose a health plan to deliver your health care benefits. The health care plan you choose makes all the difference.
