Welcome to Community First Health Plans STAR+PLUS!
We are honored to be your health plan provider as part of the Medicaid Managed Care STAR+PLUS Program. We work closely with the Texas Health and Human Services Commission (HHSC) and many doctors, clinics, and hospitals to ensure you receive the health care you need.
We’ve created this page to help address your questions and urgent needs as you transition from your previous health plan to Community First Health Plans.
Member ID cards and Welcome Packets
Your Member ID card and welcome packet are on the way! Look for them in your mailbox starting the week of September 1, 2024. Your welcome packet will include your Member ID card, Quick Start Guide, Member Handbook, important information about your STAR+PLUS benefits, how to access care, and the many services available to you.
In the meantime, your Medicaid ID number is also your Community First Member ID number. Your Medicaid ID number goes where you go. Look on your ID card from your previous STAR+PLUS health plan and use it to make appointments with your doctors, get prescriptions, and even create your Community First Member portal account. You can sign up online or download our mobile app.
Filling Prescriptions
Are you trying to fill a prescription but don’t know how? Call your pharmacy and give them your Medicaid ID number. If you have not yet received your Community First Member ID card, you can use the same Member ID number that is on your previous STAR+PLUS health plan ID card. Just tell the pharmacy you are now with Community First. The pharmacy can use the number to look up your benefits in our system and fill your prescription.

Provider Not In-Network
Can’t find your doctor in our provider network? Don’t worry, Community First has got you covered. We will work with your current provider to add them to our network so that you don’t have to change doctors.
Authorizations
Any authorizations that were on file with your previous STAR+PLUS health plan prior to September 1, 2024, have been sent to Community First and are now in our system. If you have upcoming procedures or services, you can keep your appointments.
Our highest priority is that you do not experience any interruption in your care. Community First will work with your doctors as well as with your previous health plan to ensure you receive the care you need.
Service Coordination
Every STAR+PLUS Member has their own Service Coordinator assigned to them. Service Coordinators work to ensure STAR+PLUS Members receive all the health care and social services they need to live as comfortably as possible.
In the coming weeks, you will receive a letter in the mail that provides you with the name and contact information of your assigned Service Coordinator. You will also receive a welcome call from them to introduce themselves to you and your family.
If you would like to know immediately who your Service Coordinator is, you can create a Member Portal account using your Medicaid ID number. Once you have created your account, click on “My Health Plan” and select “Service Coordinator” from the drop down. Your Service Coordinator’s information will then display on the screen.
Transportation Assistance
Do you need non-emergency medical transportation (NEMT) assistance? If you are a Community First STAR, STAR Kids, or STAR+PLUS Member and have no means of transportation or can’t get a ride to a health care appointment, you can get NEMT services at no cost.
