Community First Health Plans is currently conducting its annual Provider Satisfaction Survey as part of ongoing quality improvement efforts. A random sample of Providers will be selected to participate.
The survey gathers feedback on your experience working with Community First and helps identify areas for improvement. If selected, you may be contacted by email, mail, or telephone. A physician, office manager, nurse, or designated staff member may complete the survey.
Results from the survey will be used to guide administrative and operational changes resulting in better service for you. We appreciate your participation.
