Care Management

Community First Care Management is designed to help Members who require health care services from several different providers in order to manage their chronic condition. Our goal is to ensure that these Members have quick and easy access to needed health care services.

 

 

 

Community First Care Managers work with you, your family members, your doctors, and other members of your health care team to make sure you’re getting the care you need when you need it.

Community First Care Managers Can:

  • •   Help you understand your condition, planned tests and procedures, and treatment options
  • •   Answer questions about your health benefits
  • •   Determine your needs
  • •   Coordinate care with your doctors and therapists
  • •   Develop a plan of care and update as needed
  • •   Help order special equipment and supplies
  • •   Coordinate behavioral health services
  • •   Initiate home health care
  • •   Assist with access to community programs
  • •   Track your progress over time or changes in your condition
  • •   Evaluate additional needs

 

Special/Chronic Health Conditions Care Management Can Assist With Include:

High-Risk Pregnancy:  Support and education to help you to a full-term delivery while minimizing the risks to you and your baby.

Neonatal Care: Monitoring of premature infants admitted to the neonatal intensive care unit and up to one year of support for parents following the baby’s discharge from the hospital.

Organ Transplant: Coordination of services between you and your providers both before and after an organ transplant.

Oncology: Coordination of services between you and your providers from  diagnosis, throughtreatment, and in recovery.

Emergency Room Intervention: For Members who visit the emergency room frequently, a review of the reasons for your emergency care and options for care in other health care settings, like urgent care or your PCP.

Serious Illness: Coordination of all care and services between you and your providers.

Behavioral Health or Substance Use: Assistance finding a behavioral health provider or substance use services and coordination of all necessary care and services between you and your providers.
