Community First Health Plans (Community First) uses the following credentialing applications:
Individual Providers use the Texas Standardized Credentialing Application (v. 01/07).
Community First uses CAQH, an application repository for individual provider applications. The repository is at no cost to providers and enables the provider to keep all of their credentialing information in one location, with prompts by CAQH to update for freshness of the requirements.
Facility Providers use the Facility/Ancillary/Long-term Care Provider Credentialing Application.
This was developed with the help of TAHP and is used by the state-mandated CVO, Verisys. The facility application has a separate addendum for facility providers with multiple locations.
Community First Credentialing uses the state-mandated CVO, Verisys (formerly known as Aperture). Verisys performs Primary Source Verification for all of our applications. Community First communicates with Verisys daily via electronic start work files and by downloading completed applications.
