At Community First, we work hard to connect members of our community to health coverage through programs like Medicaid (STAR and STAR Kids) and CHIP. We believe that all adults, families, and children deserve access to high-quality care.

If you have patients who need to reapply for Medicaid or CHIP, Community First can help. We can now connect patients with in-home, one-on-one help with their Medicaid/CHIP application or renewal form. 🏡

To make an appointment, patients can

  1. Fill out our appointment form online. Go to CommunityFirstHealthPlans.com/Community-Office and select “In-Home Visit” under “Appointment Type.”
  2. Contact our Connecting Kids to Coverage team at 210-358-3350

IMPORTANT: The eligibility of ALL individuals who have Medicaid/CHIP – including children – is now under review by HHSC. Please encourage patients to reach out to us if they need assistance. We can help.

Beginning September 1, 2024 Community First Health Plans, Inc. will be adding STAR+PLUS to its line of health care products.
