IMPORTANT: If you receive a yellow envelope from Texas Health and Human Services Commission (HHSC) with a renewal form or request for information and you do not respond in the time period provided, you will lose Medicaid/CHIP.

If this has happened to you or your child and you believe you still qualify, you have 90 days to re-apply without experiencing a gap in coverage.

How to re-apply:

  1. Return your renewal form or requested information to HHSC within 90 days of the last day of the last benefit month your coverage ended.
  2. If you do not have a renewal packet or if you’re unsure what information HHSC needs, visit YourTexasBenefits.com and click “Apply” to start a new application. The renewal link will not be active during the 90-day reconsideration period.
  3. You can also call 2-1-1 and select option 2 to re-apply over the phone.
  4. If you need help, we can assist you in English or Spanish. Request an appointment at our Avenida Guadalupe Community Office to get help. We can also connect you with in-home assistance.

Beginning September 1, 2024 Community First Health Plans, Inc. will be adding STAR+PLUS to its line of health care products.
